Conclusions
The absence of corpora lutea stimulation
results in low steroids levels and
is associated with a decreased probability of pregnancy after fresh embryo transfer
The supplementation of the lutea phase with LH activity (mainly hCG) has been shown to maintain the probability of pregnancy,
albeit with a cost of severe OHSS in a proportion of patients
Conclusions
If no further stimulation of corpora lutea is performed after GnRH agonist triggering the probability of severe OHSS is virtually zero
This is the basis for the “freeze all” approach after GnRH agonist triggering that ensures maximum patient safety with efficacy,
by deferring embryo transfer of all cryopreserved embryos
in subsequent frozen-thawed cycles
