A meta‐analysis of 2017: ten RCTs did not found evidence of effect of inositol on miscarriage in women with PCOS.
A RCT on women with a parent with T2DM reported reduced GDM incidence with myo‐inositol treatment during pregnancy
Only one retrospective study confirmed this data in women with PCOS.
The effect of myo-inositol on pregnancy outcomes
in women with PCOS is still unclear.

CONCLUSIONS
The multifaceted metabolic alterations in PCOS women concur to an higher risk of maternal complications during pregnancy
Significant adverse pregnancy outcomes in PCOS are miscarriage, gestational diabetes, hypertensive disorders, preterm delivery, caesarean section
The PCOS metabolic derailment during pregnancy is associated with adverse neonatal outcomes and long term health consequences in the offspring
If women with PCOS may deserve specific management and treatment during pregnancy still need to be clarified
