 A meta‐analysis of 2017: ten RCTs did not found evidence of effect of inositol on miscarriage in women with PCOS.
 A RCT on women with a parent with T2DM reported reduced GDM incidence with myo‐inositol treatment during pregnancy
 Only one retrospective study confirmed this data in women with PCOS.

The multifaceted metabolic alterations in PCOS women concur to an higher risk of maternal complications during pregnancy
Significant adverse pregnancy outcomes in PCOS are miscarriage, gestational diabetes, hypertensive disorders, preterm delivery, caesarean section
The PCOS metabolic derailment during pregnancy is associated with adverse neonatal outcomes and long term health consequences in the offspring
If women with PCOS may deserve specific management and treatment during pregnancy still need to be clarified
