Take Home Messages
 The efficiency of FSH action is regulated by three main factors—the serum levels of circulating hormone, its intrinsic bioactivity and the efficacy of FSHR signal transduction in testicular and ovarian target cells
 Acidic FSH isoforms are more potent in stimulating oestrogen production, are more stable and have a longer half life and are associated with higher follicular thresholds. Stimulation protocols using acidic isoforms of FSH last longer.
 Different FSH preparations differ considerably. Urinary FSH are more acidic compared to recombinant FSH preparations. Even within recombinant families differences exist in acidity and biopotency
 FSH Receptor polymorphisms as well as those in the FSH-β chain are associated with different endogenous FSH and LH serum levels. Similarly, women with the least sensitive receptors and FSH ligands do need more FSH in stimulation protocols for ART
 Urinary FSH preparations seem to be as effective as recombinant FSH preparations in terms of clinical and ongoing pregnancy rates.
 Sequential regimens starting with more acidic u-FSH isoforms and switching to more basic r-FSH isoforms seem to be more effective compared to conventional COH protocols using r-FSH only
